Per Diem/Contingent Physical Therapist – Skilled Nursing Facility
Location: Dearborn
Schedule: Per diem
Join our team at Allegria Village, where we are committed to providing exceptional, compassionate care to our residents. We are currently seeking a dedicated and motivated Per Diem/Contingent Physical Therapist to join our Skilled Nursing Facility team.
Key Responsibilities
Provide direct patient care and perform thorough functional evaluations to develop individualized treatment plans
Design, implement, and adjust plans of care to achieve optimal patient outcomes, incorporating family participation when appropriate
Ensure all therapy practices align with established standards of care and facility policies
Supervise and guide support staff to help patients meet their functional goals
Collaborate effectively with the interdisciplinary care team to promote positive outcomes
Maintain compliance with facility and departmental policies, including quality improvement, safety, and infection control protocols
Complete and maintain accurate documentation, including evaluations, goals, plans, and physician prescriptions
Participate in continuing education, workshops, and professional development activities to enhance skills and stay current with best practices
Foster positive relationships with patients, families, and colleagues to ensure a supportive care environment

Education & Certification Requirements
Graduate of an accredited Bachelor’s, Master’s, or Doctorate program in Physical Therapy
Current license in good standing with the State Board of Physical Therapy Examiners
Qualifications
Knowledge and skills specific to the Skilled Nursing Facility (SNF) population
Previous SNF experience preferred
Understanding of all applicable state, federal, and professional standards
Strong problem-solving, organizational, and communication skills (both verbal and written)
Demonstrated compassion, empathy, and professionalism when working with residents and families
Excellent customer service orientation and ability to work collaboratively in a team environment

Ability to lift a minimum of 40 lbs.
Frequent walking, standing, pushing, pulling, reaching, crouching, stooping, bending, and stretching

Ability to transfer patients safely and perform required documentation and computer tasks
80-bed skilled nursing facility (smaller, team-oriented environment)
Unlike facilities that outsource therapy services, our rehabilitation team is fully in-house, promoting consistency and communication.
Lower-acuity care setting (non-vent, non-trach facility)
Continuity of care across Independent Living, Assisted Living, and Skilled Nursing on one campus
Staff build long-term relationships with residents across levels of care
Each resident has a private room (no shared rooms), supporting dignity and comfort
Convenient central location with easy access via multiple major highways
