The Care Manager is responsible for coordinating delivery of services to guests in collaboration with the facility’s team members.

Responsibilities

• The Care Manager monitors and documents the effectiveness of treatment provided, coordinates the admission process and determines with the interdisciplinary team (IDT) appropriate post discharge services.
• Coordinates and updates Care Plan. Attends meeting if requested by guest or deemed appropriate by IDT.
• Participate in daily IDT meeting, provide clinical review as necessary.
• Complete assigned sections of Discharge Plan Instructions. Review clinical post discharge instructions with guest upon discharge.

Qualifications

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a
graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN through
the State Board of Nursing.
• Must have a minimum of two (2) years’ experience in a hospital, nursing care facility, or other
related health care facility.
