Care Delivery Manager (RN) – MLTC

Location: Bronx, NY
Schedule: Monday–Friday | 8:30 AM – 5:00 PM
Employment Type: Full-Time | Temp-to-Perm Opportunity

We are seeking an experienced Registered Nurse Care Delivery Manager to support high-quality care delivery within a Managed Long Term Care (MLTC) program in the Bronx.

This is an onsite, site-based leadership role (not a field/home visit position).

This position is ideal for an RN with a strong background in MLTC, community health, or home care, along with proven management or supervisory experience.


Key Responsibilities
  • Oversee development and execution of individualized care plans to improve participant outcomes
  • Lead and participate in Interdisciplinary Team (IDT) meetings and care coordination
  • Supervise and support clinical staff and care coordination teams
  • Monitor clinical documentation, compliance, and quality of care delivery
  • Review and oversee UAS assessments and care planning accuracy
  • Ensure adherence to federal/state regulations and organizational standards
  • Manage staffing, team assignments, and operational coverage across the site
  • Coordinate transitions of care across settings (home, rehab, nursing facilities, etc.)
  • Investigate incidents and ensure timely resolution and documentation
  • Partner with leadership and HR on hiring, onboarding, coaching, and performance management

Must-Haves
  • Active NYS Registered Nurse (RN) license
  • Must have recent Managed Long Term Care (MLTC) experience – candidates without MLTC will not be considered
  • Strong background in community health and/or home care
  • Prior leadership or management experience (supervisory required)
  • Experience leading IDT meetings and care coordination
  • Knowledge of UAS assessments and regulatory compliance
  • Strong communication, organizational, and leadership skills

Nice-to-Haves
  • Experience within PACE or MLTC organizations
  • Prior role as Care Manager Lead, Clinical Manager, or Nurse Supervisor
  • Bilingual (Spanish) a plus
