Job Description
The Home Health Physical Therapist Assistant in the Northern AZ territory delivers specialized physical therapy support to patients in their homes, emphasizing mobility improvement, rehabilitation, and enhanced quality of life. Collaborating as part of a medium multidisciplinary team, this role requires daily travel within the service area and reports directly to a Physical Therapist. Responsibilities
- Conduct patient assessments to determine therapy needs
- Assist in developing and implementing individualized treatment plans
- Provide mobility training to promote patient independence
- Document patient progress and update care plans accurately
- Educate patients and families on recovery techniques and safety
- Coordinate care with multidisciplinary team members
- Evaluate patient safety and recommend necessary equipment
- Perform follow-up visits to monitor and adjust treatments
Preferred Qualifications
- Minimum 1 year of experience as a physical therapy assistant
- Current state Physical Therapist Assistant license
- Strong skills in patient assessment and rehabilitation techniques
- Expertise in mobility training and therapeutic exercises
