Job Summary:
We’re looking for a Registered Nurse (RN) Care Manager who is passionate about patient-centered care and improving the lives of those with chronic conditions. This clinic-based role focuses on education, proactive care planning and ensuring smooth transitions for patients recently discharged from the hospital.
If you’re ready to step into a meaningful, non-bedside RN role with regular hours and the chance to build lasting relationships with your patients, this position is for you.
Responsibilities:
- Educate and support patients with chronic illnesses (e.g., diabetes, hypertension, COPD)
- Create personalized care plans and help patients manage their health independently
- Lead transitions of care (TOC) by contacting recently discharged patients, reviewing discharge instructions, scheduling follow-ups and identifying any red flags early
- Coordinate referrals, home care and specialty services
- Act as a liaison between patients, families, providers and external care teams
- Assist with medication prior authorizations and patient assistance applications
- Help the practice meet quality improvement and value-based care goals
Qualifications:
- Current RN license in the state of Michigan (Required)
- Experience in care management, outpatient nursing or case management preferred
- Strong communication, critical thinking and organizational skills
- Comfortable with EMRs and working in a dynamic clinic environment
- Team player who can also manage responsibilities independently
- Experience in primary care or internal medicine
- Knowledge of value-based care models
- Familiarity with community health resources and patient support programs
