We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team at Kadima Rehabilitation & Nursing at Mt. Lebanon, a state-of-the-art skilled rehabilitation facility located in Mt. Lebanon, Pennsylvania. As an LPN with our team, you will play a vital role in delivering high-quality patient care to individuals recovering from illness, injury, or surgery.
About the Role:
As a Licensed Practical Nurse, you will:
Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and assisting with daily living activities
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor patient progress, report changes and concerns to the healthcare team, and make adjustments to care plans as needed
Provide education and support to patients and their families to promote optimal health outcomes
Participate in quality improvement initiatives to ensure the highest level of patient care and satisfaction
Responsibilities:
Conduct patient assessments, develop and implement individualized care plans, and monitor patient progress
Administer medications, wound care, and other treatments as prescribed by physicians
Assist with daily living activities, such as bathing, dressing, and grooming
Collaborate with the interdisciplinary team to provide comprehensive patient care
Maintain accurate and timely documentation of patient care and progress
