Licensed Practical Nurse, Long Term Care, Middlebury, Vermont — Start 08/27/2026 for a 4-week travel assignment, 3x12-hour night shifts in a 98-bed post-acute care unit within a long-term care facility. Weekly pay ranges from $1,378 to $1,451. This is a travel - temporary assignment with no guaranteed hours. License on file at time of submission.

Responsibilities
- Provide direct nursing care to post-acute skilled residents per the planned care pathway
- Administer prescribed medications and document in the EPIC system; monitor patient responses
- Monitor vital signs and clinical status; promptly communicate changes to the supervising RN or charge nurse
- Assist residents with activities of daily living and support rehabilitation/therapeutic activities
- Collaborate with the interdisciplinary team to update care plans and participate in rounds as needed
- Adhere to safety and infection control protocols; support appropriate patient transfers and documentation
Requirements
- Active LPN license in Vermont; license on file at time of submission
- BLS certification
- Minimum 1 year of clinical experience, preferably in long-term care or skilled nursing
- Prior travel experience required; EPIC experience within the past 2 years is required
- IV certification preferred for LPNs
- Dementia-capable care certification preferred
- Ability to work 3x12-hour night shifts; floating required; scheduling may vary; must be available to work holidays (all holidays)
- Must have reliable transportation; must be able to start within the next week as requested in notes
