- Evaluate, plan, and implement individualized treatment programs.
- Collaborate with the care team to ensure comprehensive patient care.
- Document patient progress and adjust treatment plans as needed.
- Provide education to patients and families regarding therapy goals and exercises.
Requirements:
- Current FL PT license
- Minimum 1 year of PT experience (SNF experience preferred)
- Current BLS certification (American Heart Association)
- Strong clinical skills and ability to work independently
