Licensed Practical Nurse (LPN) – Long-Term Care
Location: Wayne, NJ
Schedule: Full-Time
Available Shifts: 7:00 AM–3:00 PM | 3:00 PM–11:00 PM | 11:00 PM–7:00 AM
Every Other Weekend Required
We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide exceptional nursing care in a skilled nursing and long-term care setting. This facility utilizes paper documentation, so candidates must be comfortable with handwritten charting and accurate clinical documentation.
Must Haves- Active New Jersey LPN License in good standing
- Minimum of 1 year of Long-Term Care, Skilled Nursing, Rehabilitation, or Sub-Acute experience
- Current BLS Certification
- Comfortable working in a paper charting/documentation environment
- Strong medication administration and clinical assessment skills
- Ability to identify, assess, and report changes in resident condition
- Excellent communication, organizational, and time management skills
- Ability to work effectively as part of an interdisciplinary team
- Ability to work every other weekend
- Compassionate, dependable, and committed to providing exceptional resident care
- Experience with wound care and treatment management
- IV Certification (where applicable)
- Experience caring for rehabilitation, sub-acute, or memory care residents
- Previous Charge Nurse or leadership experience
- Flexible availability to pick up additional shifts
- Administer medications and treatments as ordered by the physician
- Monitor, assess, and document resident conditions using paper medical records
- Collaborate with physicians, RNs, and the interdisciplinary team to develop and implement individualized care plans
- Provide high-quality nursing care while promoting resident dignity and independence
- Maintain complete, accurate, and timely handwritten documentation
- Educate residents and families regarding medications and care plans
- Ensure compliance with facility policies and state and federal regulations
- Maintain a safe, clean, and supportive environment for residents
