Compensation$68.00–$72.00 per hour (based on experience)
Competitive pay based on supervisory experience, home care background, and clinical leadership expertise.

8:30 AM – 5:30 PM
1-hour unpaid lunch
No routine weekends
Field-based home visits
Diagnostic & Treatment Center (DTC)
Medical Practice
Active New York State Registered Nurse (RN) license
Previous Home Care or Certified Home Health Agency (CHHA) experience
Supervisory or clinical leadership experience
Experience conducting nursing supervisory visits and competency evaluations
Strong knowledge of home care regulations and clinical documentation
Excellent assessment, communication, and organizational skills
Valid New York State driver's license
Reliable personal vehicle for travel throughout assigned territory
Authorized to work in the United States
Experience providing staff education and clinical in-services
Quality assurance or performance improvement experience
OASIS knowledge
Case management or care coordination experience
Bilingual skills
Conduct supervisory joint visits with Field Nurses in patients' homes
Mentor, coach, and support clinical field staff
Provide orientation, training, and ongoing clinical education
Monitor quality of care and ensure regulatory compliance
Perform clinical nursing duties in the Diagnostic & Treatment Center (DTC) or Medical Practice as needed
Review documentation for accuracy and completeness
Identify opportunities for clinical improvement and best practices
Collaborate with interdisciplinary teams to ensure excellent patient outcomes
Excellent hourly compensation
