Physical Therapist – Brooklyn, NY

Location: Brooklyn, NY (Stillwell Avenue)
Setting: On-site (Orientation in the Bronx, NY)
Schedule: Monday – Friday | 9:00 AM – 5:00 PM
Position Type: Temporary Assignment (LOA Coverage through September 25, 2026)

About the Opportunity

We are seeking a compassionate and skilled Physical Therapist to join a multidisciplinary healthcare team providing high-quality rehabilitative care. In this role, you will evaluate participants, develop individualized treatment plans, and provide therapeutic interventions designed to restore function, reduce pain, improve mobility, and enhance overall quality of life.

Must Haves
  • Doctorate or Master's Degree in Physical Therapy from an accredited program
  • Current New York State Physical Therapist license
  • Minimum of 1–2 years of clinical Physical Therapy experience
  • Experience evaluating patients and developing individualized treatment plans
  • Strong knowledge of therapeutic exercise, functional mobility training, gait training, balance, and fall prevention
  • Excellent communication and documentation skills
  • Ability to collaborate effectively with an interdisciplinary healthcare team
  • Proficiency with electronic medical records (EMR)
Nice to Have
  • Experience working with geriatric or adult populations
  • Experience in PACE, outpatient rehabilitation, home care, or skilled nursing settings
  • Knowledge of adaptive equipment and assistive devices
  • Bilingual (English/Spanish or other languages)
  • Experience with fall-risk assessments and chronic disease management
Responsibilities
  • Evaluate participants and establish individualized treatment goals
  • Develop and implement evidence-based physical therapy treatment plans
  • Provide therapeutic exercises, manual therapy, gait training, balance training, and functional mobility interventions
  • Monitor participant progress and modify treatment plans as needed
  • Educate participants and caregivers on home exercise programs and injury prevention
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals
  • Maintain accurate and timely clinical documentation
  • Ensure compliance with all regulatory, professional, and organizational standards
