We are seeking a dedicated and compassionate Social Worker to join our team, a 235-bed facility located in Berkeley Township, New Jersey. As a Social Worker at our facility, you will play a vital role in providing long-term skilled nursing care and short-term post-acute rehabilitation to our patients.

About the Role:

As a Social Worker, you will work closely with our interdisciplinary team to provide high-quality care and support to our patients. Your responsibilities will include:

• Conducting assessments and developing care plans to meet the psychosocial needs of our patients

• Providing counseling and support to patients and their families to address emotional, social, and psychological issues

• Facilitating communication and coordination between patients, families, and healthcare providers

• Identifying and addressing social determinants of health that impact patient outcomes

• Collaborating with other healthcare professionals to develop and implement individualized treatment plans

Responsibilities:

• Provide psychosocial assessments and develop care plans to meet the needs of our patients

• Develop and implement individualized treatment plans in collaboration with other healthcare professionals

• Provide counseling and support to patients and their families to address emotional, social, and psychological issues

• Facilitate communication and coordination between patients, families, and healthcare providers

• Identify and address social determinants of health that impact patient outcomes

Requirements:

• Bachelor's or Master's degree in Social Work (MSW) from an accredited program

• Minimum of 2 years of experience in a healthcare setting, preferably in a social work role

• Strong communication, assessment, and intervention skills

• Ability to work effectively in a fast-paced environment and prioritize tasks efficiently

• Strong knowledge of social work principles, theories, and practices
