MDS Coordinator Job Qualifications:

  • Must have 1 year experience as an MDS Coordinator.

  • Must have valid and current nursing license, in good standing with the state of Wisconsin.

  • This role requires on-site presence at our facility for the first 90 days. After this initial period, a hybrid schedule of 3 days on-site and 2 days remote may be available, based on performance and business needs.

  • Licensed Practical Nurse (LPN) is acceptable, Registered Nurse (RN) preferred.

  • Proficient with use of computers and computer systems such as Microsoft Outlook and Office.

  • PCC knowledge and experience is preferred.

MDS Coordinator Job Description:

The MDS Coordinator will join the facility’s nursing leadership team and will be responsible to assess and monitor proper treatment for residents in nursing homes. The MDS Coordinator determines the health status, care plans, and procedures for intake of residents according to state and federal standards. to code experience.

MDS Coordinator Job Responsibilities:

  • Understand MDS 3.0 terminology to help master the entire MDS process and reimbursement system.

  • Execute accurate and timely completion of all Medicare/Medicaid MDS processes to ensure appropriate reimbursement for services provided.

  • Use strong assessment skills and be detail-oriented with MDS completion.

  • Coordinate with the Interdisciplinary Team to ensure MDS compliance, understanding, and follow-through, as well as to discuss appropriate programming to address patient restorative needs.
