Northwest Habilitation Services- Hub Lead

We are looking for a Hub Lead position for our NE Portland location!

To be committed to helping clients with persistent mental illness overcome the challenges of daily living through a support-based role. As a Hub Lead, your job is to help our clients learn the skills needed to live and succeed in their community. You will work in group housing setups, and very occasionally individual households, while also providing support to other staff members at your location.

Hours: Variable- 30+/wk.

Pay: 21 an hour

Job Duties: Use a Person-Centered approach utilizing a Support Plan (PCSP) tailored to the specific needs of the individual receiving services.

· Set goals that are real and achievable

· Engage with our clients to build trust

· Establish routines with clients to build self-confidence

· Help develop calendaring skills to stay on track

· Hygiene reminders and why it’s important

· Housekeeping reminders

· Teach how to do laundry

· Meal preparation and menu planning

· Grocery shopping

· Help develop knowledge regarding money management

· General health and safety queuing

· Encouraging social inclusion and communication

· Assisting the client with public transit

· Provide support and coaching to staff as needed as well as communicate scheduling needs to the office.

· Teach and assist clients in accessing community resources such as food boxes, healthcare appointments and other services as appropriate.

· Maintain confidentiality of records relating to clients. Meets standards for documentation and accuracy of work.

· Attend any required trainings or meetings as set by Management.

Job Requirements:

  • 18 years or older (required)

  • High school diploma or GED (required)

  • Clear background check (required)

  • Experience in a mental health setting (preferred)
