Job Summary:

Alpine Care of Zion is seeking a compassionate and skilled Social Service Designee to join our team in Zion, IL. Responsibilities:

• Conduct initial assessments and develop personalized care plans to meet the social and emotional needs of residents

• Provide counseling and support to residents, their families, and care staff to address concerns, needs, and concerns

• Collaborate with interdisciplinary teams to develop and implement resident-centered care plans

• Maintain accurate and detailed records of resident interactions, assessments, and care plans

• Provide education and guidance to care staff on social and emotional support strategies

• Participate in staff training and development programs to enhance knowledge and skills

Requirements:

• 1 year of experience in social work, counseling, or a related field

• High school diploma or equivalent required; will train the right candidate

• Possession of current professional licensure/registration as a Social Worker (LSW/LCSW) or a related field (RSA, RN, etc.) (if applicable)

• Excellent communication, interpersonal, and organizational skills

• Ability to work effectively in a fast-paced environment with multiple priorities

• Proficiency in electronic medical records (EMRs) and other software applications
