Responsibilities
- Plan, organize, and direct in-home nursing care to meet patient needs and goals
- Conduct initial and ongoing comprehensive nursing assessments and document findings
- Develop, implement, and update the patient’s plan of care in collaboration with the care team
- Educate patients and families on nursing care, treatment options, and rights; provide learning materials as appropriate
- Coordinate patient care across disciplines and ensure timely, accurate documentation and communication
- Monitor patient status, adjust plans as needed, and ensure safe, quality care delivery

Requirements
- Active RN license in the state of practice (California) required
- CPR or BLS certification required
- Minimum two years of recent nursing experience in field settings (home health, case management, or acute care)
- Bachelor’s Degree in Nursing (BSN) preferred
- Valid driver’s license (21+), current automobile insurance, and reliable transportation
- Ability to perform comprehensive home-based nursing assessments and to work independently within home care settings
