We are offering a rewardingfull-time Vocational Case Managerposition to provide case management services while working with our Adult Services team.
ResponsibilitiesThis position serves as a key member of the Mental Health Case Management team and is responsible for assessing, developing, and implementing vocational goals for individuals with severe mental health challenges. This position performs work duties specific to the I-Work program while maintaining a reduced Mental Health Case Management caseload. The successful candidate will demonstrate excellent communication and organizational skills, be task and goal oriented, and require minimal supervision. This highly collaborative role partners with team members, community organizations, and employers to create vocational opportunities that support clients in achieving employment and meaningful participation in the community.
Required Education and Experience- Bachelor's degree in a human services or related field.
- Candidates must be registered as a QMHP with the Virginia Board of Counseling.
- One (1) year of related experience providing community-based mental health and case management services for individuals with Serious Mental Illness (SMI) and Substance Use Disorders (SUD), including counseling and crisis intervention.
- All GRCSB employees providing case management services must complete the DBHDS Case Management Curriculum within 30 days of employment.
- Valid driver's license with a DMV point balance of -6 or higher.
- Knowledge of and experience with vocational rehabilitation interventions, assessments, and service delivery.
- Strong commitment to supporting individuals with severe and persistent mental illness in living independently, obtaining competitive employment, and accessing continuous community supports.
- Ability to establish supportive, trusting relationships with individuals experiencing severe and persistent mental illness while respecting client rights and treatment preferences.
- Excellent written, verbal, and interpersonal communication skills.
- Ability to effectively consult with and connect clients to emergency and crisis intervention services.
- Ability to exercise sound judgment and demonstrate initiative.
- Ability to maintain cooperative and effective working relationships with Greater Reach staff and community partners.
- Knowledge of laws and regulations governing case management services.
- Proficiency with basic computer applications and technology.
- Three (3) years of related experience providing community-based mental health and case management services for individuals with SMI and SUD, including counseling and crisis intervention.
Ability to lift up to 35 pounds (e.g., meeting supplies, deliveries, office materials). Occasional evening work may be required to support board or agency functions. Travel throughout the Commonwealth of Virginia, including overnight travel when necessary.
Pre-employment Requirements: Fingerprints, State and FBI criminal background checks, drug screening, and Central Registry clearance are required upon selection. Greater Reach Community Services Board maintains a drug-free workplace.
