Social Services Director
Location: Cherry Hill Nursing and Rehab – Westland, MI
Cherry Hill Nursing and Rehab is seeking an experienced and compassionate Social Services Director to join our skilled nursing facility in Westland, MI.
The Social Services Director serves as a liaison and advocate for residents’ social and emotional needs and is responsible for planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department.
Key Responsibilities
Develop comprehensive social assessments and individualized care plans that identify medically related social and emotional needs, realistic goals, and specific interventions
Participate actively in the pre-admission and admissions processes
Serve as a compassionate liaison and advocate for residents and their families
Listen to and respond appropriately to resident and family concerns while maintaining professionalism and confidentiality
Document residents’ behavioral, emotional, and psychosocial status accurately
Provide individual and group support to residents and families during periods of adjustment, crisis, or special need
Establish and maintain a volunteer program designed to meet residents’ social service needs
Orient residents and families to the nursing center and assist with adjustment to the new environment
Maintain compliance with applicable federal, state, and local regulations governing social services in long-term care
Qualifications
Bachelor’s degree from an accredited school in Social Work or a related field
Knowledge of social services practices, procedures, laws, regulations, and guidelines governing long-term care
Strong communication, organizational, and interpersonal skills
Compassionate and professional approach when working with residents, families, and staff
Ability to work independently and as part of an interdisciplinary team
