Lead Social Services With Purpose

A skilled nursing facility in Orlando, Florida is seeking an experienced Director of Social Services to lead the facility’s social services department and support residents throughout their stay.


This position is ideal for a compassionate, organized professional who understands the unique needs of residents in skilled nursing and long-term care. The Director of Social Services will work closely with residents, families, nursing, rehabilitation, case management, and facility leadership to support successful care planning, discharge coordination, and resident satisfaction.


Key Responsibilities

  • Lead and manage the facility’s social services department
  • Complete psychosocial assessments and maintain accurate documentation
  • Participate in resident care planning and interdisciplinary team meetings
  • Coordinate discharge planning and transition-of-care needs
  • Assist residents and families with adjustment to the facility environment
  • Address resident and family concerns in a professional and timely manner
  • Coordinate community resources, transportation, equipment, and support services as needed
  • Support residents with advance directives and related documentation
  • Monitor resident rights and advocate for resident needs
  • Assist with behavioral, emotional, and psychosocial concerns
  • Maintain compliance with federal, state, and facility requirements
  • Support survey readiness and documentation standards
  • Build strong working relationships with residents, families, staff, and outside providers


Qualifications

  • Previous social services experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting
  • Prior leadership experience preferred
  • Strong understanding of discharge planning, care planning, resident rights, and psychosocial needs
  • Excellent communication and interpersonal skills
  • Strong organization and documentation skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred
  • Social work licensure or certification preferred when applicable
