JOB SUMMARY
Livingston Health and Rehabilitation Center is seeking a Transportation & Social Services Assistant to safely, timely, and reliably transport residents to and from appointments while also supporting the Social Services Department with scheduling, coordination, and communication needs. A CNA license is preferred, but not required.
ESSENTIAL FUNCTIONS
Transportation Responsibilities
Maintain safe and courteous motor vehicle operation in compliance with applicable laws.
Perform safe resident transfers (bed to chair, chair to bed).
Safely load and unload residents to and from the transport van.
Conduct routine vehicle inspections and report any safety issues.
Report all vehicle accidents, traffic citations, and incidents immediately and complete required documentation.
Social Services Responsibilities
Assist with scheduling resident appointments and coordinating transportation logistics.
Communicate with residents, families, and healthcare providers regarding upcoming appointments.
Support Social Services staff with general administrative and resident-service needs.
Help ensure a positive resident experience through timely communication and customer service.
EDUCATION, LICENSURE, AND EXPERIENCE
Required: Valid driver’s license and has an acceptable driving record
Preferred: Certified Nursing Assistant licensure.
Preferred: Experience in Behavior Add-on Programs.
