Job Summary:
Highfield Gardens Care Center, a sub-acute and long-term care facility in Great Neck, New York, is seeking an organized Discharge Planner to join our team. As a key member of our healthcare community, you will play a vital role in ensuring a seamless transition for our residents back to their homes or alternative settings.
Discharge Planner Responsibilities:
Coordinate with multidisciplinary teams, including physicians, nurses, and social workers, to develop individualized discharge plans for residents
Assess residents' medical, social, and psychological needs to determine the most appropriate post-acute care setting
Facilitate communication with family members, care providers, and other stakeholders to ensure a smooth transition
Identify and address potential barriers to discharge, such as insurance and transportation issues
Maintain accurate records and reports on discharge planning activities and outcomes
Collaborate with internal departments, including admissions, marketing, and quality improvement, to ensure a cohesive approach to care
Discharge Planner Requirements:
Bachelor's degree in a healthcare-related field (e.g., social work, nursing, healthcare administration)
Prior experience in social work, SNF, or a related field
Strong communication, interpersonal, and problem-solving skills
Ability to work in a fast-paced environment and prioritize multiple tasks and deadlines
