Social Worker - Hospice Care

We are seeking a dedicated and empathetic Social Worker to deliver a wide range of psychosocial services, ensuring holistic care and emotional support for those in need. Key Responsibilities:

- Conduct initial psychosocial evaluations of patients and their families to assess needs and develop appropriate care plans

- Provide ongoing psychosocial counseling to support emotional well-being and coping strategies

- Deliver direct casework services, including patient and family education, advocacy, and resource coordination

- Assist with discharge planning to ensure smooth transitions and continued support post-hospitalization

- Offer bereavement services to families experiencing loss, providing comfort and guidance

- Engage in community education, outreach, and referral activities to promote awareness and access to hospice services

- Collaborate with the interdisciplinary team to develop and implement individualized plans of care

Requirements:

EDUCATION, TRAINING AND EXPERIENCE: Masters’ degree in Social Work from an accredited educational institution. LMSW or LCSW preferred. Incumbent will have one year experience in individual, family and group counseling, preferably in a healthcare or hospice setting. MSW candidates will have 6 months to obtain LMSW.

CERTIFICATION/LICENSURE: Possess appropriate certificate / license to practice as a Licensed healthcare professional in the State of New York. Certification in Hospice & Palliative Care is preferred if offered for discipline.

Valid NYS driver’s license required
