Ten Broeck Center for Rehabilitation and Nursing is seeking a Full-Time Activity Leader for our Memory Care unit. This is a Full-Time day shift position with alternating weekends.

Pay Rate: $16.00/hour
ESSENTIAL FUNCTIONS:
Assists in providing recreational and other activities as per the individualized activity interests and multidisciplinary care plan.
Leads or facilitates various activity groups. (i.e.: religious services, physical ad cognitive games, arts and crafts, gardening, grooming etc.
Communicates with residents with visual, speech, auditory, cognitive, physical and emotional impairments.
Completes Activities section of MDS forms as scheduled.
Regularly assess the social and emotional needs of the residents with the Director and set appropriate goals for meeting those needs within the overall plan of care.
Helps in keeping activity materials organized.
Helps decorate the facility with seasonal decorations.
Gives input on monthly activity calendars, and hangs them on appropriate units.
Works with the interdisciplinary team, attending various weekly care plan meetings.
Reports all cases of alleged violations involving mistreatment, neglect or abuse, including injuries of unknown sources.
Assist with transportation, if needed, to and from activities.
Assist resident to participate in individual activities of choice; i.e.: set up of equipment, positioning assistance, providing sufficient lighting and space.
Inform residents of various activities available to them even when Activities staff are not available.
Assures safety of residents at all times.

PHYSICAL REQUIREMENTS:
Transporting residents to and from in -house activities and accompanying residents to and from community programs.
Position requires movement throughout the entire building.
Pushes 225 lbs. in a wheelchair, recliner, or Geri - chair.
Leading appropriate exercise classes (Pushing, bending, stretching)
