Job Summary/Purpose:
The Case Manager provides client-centered, trauma-informed, and culturally responsive services to individuals and families, supporting the mission ofServiciosde La Raza. This position exercises independent judgment and discretion in assessing client needs, developing comprehensive care plans, and coordinating services. The Case Manager provides psychoeducation, crisis intervention, and harm reduction support, whilemaintainingcompliance with agency standards and regulatory requirements. By engaging with internal and external partners,maintainingaccuratedocumentation, andparticipatingin community outreach, the Case Manager strengthensequitableaccess to services and enhances client well-being.
This role supports survivors of crime on their path to safety, healing, and self-sufficiency. The Victim Supportive Services program offers comprehensive, trauma-informed careincluding crisis intervention, mental health support, case management, advocacy, safety planning, and skill-building. The program provides confidential, inclusiveassistanceregardless of police involvement, ensuring every survivor has access to the resources they need to recover and rebuild.
JobDuties andKeyResponsibilities:
- Assess clients individualized needs via a comprehensive multifaceted assessment tool addressing social determinants of health
- Develop comprehensive case management plans and coordinate care, applying professional judgment to prioritize and tailor services
- Facilitate client-centered, trauma-informed interventions and harm reduction approaches
- Provide psychoeducationregardingissues relevant to mental health and substance misuse
- Develop client safety plans as needed
- Engage in de-escalation and crisis interventions whenrequired, exercising discretion in response strategies
- Participate in targeted community outreach and events across diverse settings
- Provide internal and external resources and referrals to support client care
- Complete all necessary client and department documentation according to SDLR standards
- Maintaintimelyand detailed records of all client interactionsin accordance withbest practices
- Uphold confidentiality of allclientprotected health information following HIPAA guidelines
- Engage with external community-serving organizations andestablishnew partnerships
- Exercise discretion and independent judgment in decision-making, service prioritization, and problem resolution
- All members of the SDLR workforce are expected to support the agencys community outreach, civic engagement, and recruitment efforts
- Support the agencys fundraisingobjectives, which may include tasks such as active fundraising, grant development, solicitation of donations, agency ambassadorship, and informational outreach, and public speaking
- Commitment to and experience with equity, cultural relevance, diversity, social justice, and disparity reduction practices with persons of color, agencies of color, and underserved communities
- Ability toestablishand maintain effective working relationships with clients, co-workers, agency partners, and the public
- Other duties as assigned
Required Education,experienceand Credentialing:
- Valid Drivers License and reliable transportation
- High School Diploma or equivalent
- Bilingual (English-Spanish)
Preferred Education,experienceand Credentialing:
- Bachelors degree in psychology, human services, social work, orotherrelated field
- Relevant years of experience will be considered
