Your Role

As a Medical Administrative & Patient Outreach Coordinator, you are a key part of the patient experience. You’ll:

  • Schedules/Cancel appointments

  • Coordinate care with external agencies, family, payer sources, and other services as needed

  • Manage files, review progress notes written from other members of the treatment team

  • Provide resources, information and referrals to patients as needed

  • Help coordinate patient care

  • Participate in patient-related meetings and trainings

  • Actively monitoring secure texting platform for patient communication

  • Coordinate care that is safe, timely, effective, efficient, equitable, and client-centered

  • Handle case assignments, review case progress

  • Help patients achieve wellness and program compliance

  • Facilitate multiple care aspects (case coordination, information sharing, reporting etc.)

  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options

  • Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction

Requirements

  • Effective communication skills

  • Excellent organizational and time management skills

  • Problem solving skills and ability to multi-task

  • Compassionate and sympathetic

  • Able to work well with a team

  • Beneficial but not essential: experience in medical field.
