Your Role
As a Medical Administrative & Patient Outreach Coordinator, you are a key part of the patient experience. You’ll:
- Schedules/Cancel appointments
- Coordinate care with external agencies, family, payer sources, and other services as needed
- Manage files, review progress notes written from other members of the treatment team
- Provide resources, information and referrals to patients as needed
- Help coordinate patient care
- Participate in patient-related meetings and trainings
- Actively monitoring secure texting platform for patient communication
- Coordinate care that is safe, timely, effective, efficient, equitable, and client-centered
- Handle case assignments, review case progress
- Help patients achieve wellness and program compliance
- Facilitate multiple care aspects (case coordination, information sharing, reporting etc.)
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
