This is an onsite/ non-remote position.

As our MDS Coordinator, you will play a vital role in ensuring accurate assessments, regulatory compliance, and quality resident outcomes. This is an excellent opportunity for a detail-oriented nurse who enjoys working closely with an interdisciplinary team and making a meaningful impact every day.

Responsibilities:

  • Complete and coordinate MDS assessments in accordance with state and federal regulations.

  • Ensure accurate and timely completion of care plans and required documentation.

  • Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.

  • Monitor clinical documentation to support appropriate reimbursement and regulatory compliance.

  • Participate in quality improvement initiatives and survey preparedness.

  • Educate and support staff regarding MDS processes and documentation requirements.

Qualifications:

  • Active LPN or RN license required. RN preferred.

  • Previous MDS Coordinator experience in a long-term care or skilled nursing setting required.

  • RAC-CT certification preferred.

  • Working knowledge of Medicare, Medicaid, the RAI process, and PDPM.

  • Strong assessment, organizational, and critical thinking skills.

  • Experience with PointClickCare (PCC) preferred.

  • Excellent communication and teamwork abilities.
