Key Responsibilities:
Conduct assessments and develop individualized service plans.
Monitor client progress and document outcomes.
Coordinate services with healthcare providers, families, and support staff.
Ensure compliance with state and federal regulations.
Advocate for individuals' rights, goals, and community inclusion.
Participate in interdisciplinary team meetings and case reviews.
Qualifications:
Bachelor's degree in a human services field (e.g., psychology, social work, special education, or related area).
Experience working with individuals with intellectual or developmental disabilities preferred.
Strong communication, documentation, and case management skills.
Must be QIDP certified
