Key Responsibilities:

  • Conduct assessments and develop individualized service plans.

  • Monitor client progress and document outcomes.

  • Coordinate services with healthcare providers, families, and support staff.

  • Ensure compliance with state and federal regulations.

  • Advocate for individuals' rights, goals, and community inclusion.

  • Participate in interdisciplinary team meetings and case reviews.

Qualifications:

  • Bachelor's degree in a human services field (e.g., psychology, social work, special education, or related area).

  • Experience working with individuals with intellectual or developmental disabilities preferred.

  • Strong communication, documentation, and case management skills.

  • Must be QIDP certified
