Hospice RN Case Manager

We are seeking a compassionate and experienced Hospice RN Case Manager to join our team in Berkshire County. As a Hospice RN Case Manager, you will be responsible for providing high-quality, patient-centered care to individuals and families in their homes and communities.
$48 to $50 an hour with weekly pay. This is an on-the-road position requiring the candidate to have reliable, insured transportation.
Responsibilities:
Plan, organize, and direct hospice care, building from the resources of the community to meet the needs of patients and families
Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
Provide professional nursing care by utilizing all elements of the nursing process
Develop and implement individualized care plans, addressing physical, emotional, social, and spiritual needs
Collaborate with patients, families, and healthcare providers to provide comprehensive care
Supervise hospice aides and act as Case Manager when assigned by Clinical Supervisor
Requirements:
Graduate of an accredited school of nursing, bachelor's preferred
Must possess an active and unencumbered RN license in Massachusetts
One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care, preferred
CPR certification
