Position Summary
The SOCIAL SERVICES DIRECTOR is responsible for planning, implementing, and directing all aspects of the facility’s social services program to meet the emotional, psychological, and social needs of residents and their families. This role ensures compliance with federal, state, and local regulations while fostering a supportive, person-centered environment that enhances residents’ quality of life. The Director serves as a leader, advocate, and resource to residents, families, staff, and community partners.
Key Responsibilities
Resident Care & Advocacy
Conduct psychosocial assessments and care planning for all residents upon admission, quarterly, and as needed.
Advocate for residents’ rights, dignity, and independence, ensuring care is person-centered.
Provide support to residents and families in adjustment and discharge planning.
Leadership & Program Oversight
Lead the social services department and ensuring high standards of professional practice.
Collaborate with the interdisciplinary team to integrate social services into overall resident care.
Monitor policies and procedures in alignment with regulatory requirements.
Compliance & Documentation
Maintain accurate and timely documentation in the medical record, including care plans, progress notes, and discharge summaries.
Prepare for and participate in surveys, audits, and QAPI initiatives.
Quality Improvement
Identify trends and develop action plans to improve psychosocial well-being and satisfaction.
Participate in QAPI meetings and contribute to performance improvement initiatives.
Excellent interpersonal, counseling, and conflict-resolution skills.
Ability to handle sensitive resident/family issues with professionalism, empathy, and confidentiality.
Preferred Competencies
Experience with behavioral health, dementia care, and trauma-informed practices.
Strong background in discharge planning and coordination with hospitals, community providers, and managed care organizations.
Familiarity with PointClickCare or other electronic health record systems.
Proven success in survey preparation, QAPI projects, and interdisciplinary collaboration.
