As a leading provider of home health care services, we are committed to delivering compassionate care to our patients in the comfort of their own homes. We are a small Home Health Agency, without the corporate clutter :-)


We ask that you will:


  • Develop treatment plans and rehabilitation programs
  • Collaborate with skilled nursing staff on care planning
  • Conduct therapeutic interventions and exercise programs
  • Assist with discharge planning


We also ask that you will accept a $1000.00 sign on bonus :-)

Hi, Primary HomeCare Specialties is nurse owned and operated since 2009 by yours truly Celeste Cefalu.
We are a small Home Health Agency located in Milton, We service Milton, Quincy, Randolph, Canton and Dorchester. Accepting case by case in Weymouth Braintree Roxbury (We love joint replacement patients and will expand a bit for these cases :-) We are a small crew and we love what we do !!! My Physical Therapist has been with us since 2009 and now wishes to retire :-(
We have an extremely talented, competent and well loved PTA who will work in collaboration with you. We are looking for a Physical therapist to conduct the initial PT eval and subsequent Re-Eval as needed. The position is part-time per visit, perfect for the Therapist who is looking for a relaxed paced work environment. Per vsit rate: $100.00 :-)
