We are seeking a dedicated and detail-oriented Compounding Pharmacist to join our dynamic healthcare team. In this vital role, you will be responsible for preparing customized medications through compounding, ensuring precise formulation and adherence to all safety and regulatory standards. Your expertise will directly contribute to personalized patient care, especially in complex cases requiring tailored medication solutions. Duties

  • Prepare compounded medications accurately following established formulas and USP 797 guidelines to ensure safety and efficacy.
  • Conduct thorough patient assessments to determine appropriate medication formulations based on individual needs.
  • Manage pharmacy inventory, including sourcing ingredients, maintaining stock levels, and ensuring proper storage of compounded substances.
  • Ensure compliance with all federal and state pharmacy laws, drug regulations, and healthcare policies related to compounding practices.
  • Collaborate with healthcare providers to optimize medication therapy plans, including medical management and pain management strategies.
  • Maintain meticulous documentation of compounding procedures, patient records, and regulatory compliance in pharmacy information systems (PIS) and electronic health record (EHR) systems.
  • Oversee pharmacy team operations, including supervising pharmacy technicians and ensuring adherence to aseptic techniques and safety protocols such as USP 797 standards.

Requirements

  • Valid pharmacist license in good standing with relevant state and federal pharmacy law knowledge.
  • Extensive experience in pharmacy operations with a focus on compounding medications; LTC (Long-Term Care) or hospital experience is highly desirable.
  • Strong understanding of medical terminology, physiology, anatomy, drug regulation, and clinical confidentiality policies such as HIPAA.
  • Proficiency with pharmacy software systems, EMR (Electronic Medical Records), EHR systems, and inventory management tools.
  • Critical care experience or practicing as a pharmacist in acute care settings is preferred; pediatric or community pharmacy experience is a plus.
  • Certifications in basic life support (BLS), immunizations, aseptic technique, and pharmaceutical regulatory compliance are highly valued.
  • Excellent customer service skills with the ability to communicate effectively with patients and healthcare providers while maintaining professionalism under HIPAA regulations.
