RN Care Manager
Job Description
We are seeking an experienced RN Care Manager to become a part of our team! You will coordinate medical and health services in hospitals, assisted living, rehab centers, in the home as well as assist in recommending and setting up medical services and care. This is a hybrid position - work part of the time at home, work part of the time at client sites!
Responsibilities:
- Oversee medical and health service activities and personnel
- Plan and coordinate health services
- Work collaboratively with physicians, care partners, and financial and legal resources to implement appropriate healthcare programs
- Establish and enforce healthcare policies and procedures
Qualifications:
- Previous experience in healthcare or other medical fields
- Assertive personality and ability to accomplish goals and objectives
- Strong organizational skills
- Excellent written and verbal communication skills
- Previous experience in Case Management, Acute Care, or other related field is preferred
