Registered Dietitian – Skilled Nursing Facility
A NYC based skilled nursing facility is seeking a compassionate and knowledgeable Registered Dietitian (RD/RDN) to provide comprehensive nutritional care to residents in a long-term care and rehabilitation setting. The Dietitian will assess residents’ nutritional needs, develop individualized care plans, monitor outcomes, and collaborate with the interdisciplinary care team to support residents’ overall health and well-being.
Key Responsibilities
- Conduct comprehensive nutritional assessments for new admissions and existing residents.
- Develop, implement, and update individualized nutrition care plans based on residents’ medical conditions, dietary needs, preferences, and goals.
- Monitor residents for weight changes, malnutrition risk, dehydration, altered nutritional status, and other nutrition-related concerns.
- Provide recommendations for therapeutic diets, nutritional supplements, texture-modified diets, and enteral nutrition as appropriate.
- Participate in interdisciplinary care plan meetings and collaborate with physicians, nursing, speech therapy, rehabilitation, social services, and food service staff.
- Review laboratory results, medications, diagnoses, weight trends, meal intake, and other clinical information affecting nutritional status.
- Monitor residents receiving tube feedings and provide appropriate nutritional recommendations.
- Complete nutrition documentation and assessments accurately and within required timeframes.
- Educate residents, families, and staff regarding nutrition, therapeutic diets, and dietary modifications.
- Work closely with Dietary/Food Service staff to ensure meals meet residents’ nutritional requirements and prescribed diets.
- Assist with identifying and addressing residents at risk for malnutrition, unintended weight loss, pressure injuries, and other nutrition-related conditions.
- Participate in quality assurance, performance improvement, and regulatory compliance initiatives.
- Ensure nutrition services comply with CMS requirements, state regulations, facility policies, and standards of care.
- Assist with survey readiness and provide documentation or support during regulatory surveys as needed.
Qualifications
- Bachelor’s degree in Nutrition, Dietetics, or a related field required.
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
- Current state licensure/certification as required.
- Previous experience in a Skilled Nursing Facility, Long-Term Care, Rehabilitation, or healthcare setting strongly preferred.
- Knowledge of geriatric nutrition, therapeutic diets, enteral nutrition, weight management, wound healing, and malnutrition preferred.
- Familiarity with MDS, care planning, CMS regulations, and long-term care documentation is highly desirable.
- Strong communication, organizational, documentation, and interdisciplinary collaboration skills.
- Ability to effectively manage multiple residents and priorities in a fast-paced clinical environment.
Preferred Experience
Candidates with experience in Skilled Nursing, Long-Term Care (LTC), Subacute Rehabilitation, Geriatrics, MDS, Nutrition Care Planning, Therapeutic Diets, Tube Feeding/Enteral Nutrition, Wound Care Nutrition, Weight Management, and CMS compliance are strongly encouraged to apply.
Must be comfortable commuting to upper Manhattan/Washington Heights area
