A Nursing Home in Collingdale, PA, is seeking a compassionate and dedicated Social Services Professional to join our team. In this role, you will play an essential part in supporting the emotional, social, and psychological well-being of our residents and their families. You will work collaboratively with residents, families, and the interdisciplinary healthcare team to ensure each resident receives comprehensive, individualized support.
ResponsibilitiesProvide emotional support, counseling, and advocacy to residents and their families.
Address residents' social, emotional, and psychological needs.
Develop and implement individualized care plans that promote resident well-being, independence, and quality of life.
Facilitate communication between residents, families, healthcare providers, and other stakeholders.
Conduct psychosocial assessments and evaluations to identify resident needs and develop appropriate interventions.
Collaborate with nursing, medical, rehabilitation, and other healthcare professionals to coordinate comprehensive resident care.
Assist residents and families with accessing appropriate community resources and support services.
Support residents and families through transitions of care, including admissions, discharges, and changes in level of care.
Document resident progress, interventions, and services provided while maintaining accurate and confidential records.
Maintain compliance with HIPAA regulations and facility policies.
Advocate for residents' rights, preferences, and overall quality of life.
Bachelor's degree in Social Work, Psychology, Human Services, or a related field, or equivalent relevant experience.
Minimum of 4 years of experience in social services or a related healthcare field.
Experience working with older adults, patients, residents, or families in a healthcare or long-term care setting preferred.
Strong communication, interpersonal, organizational, and problem-solving skills.
Ability to maintain confidentiality and comply with HIPAA requirements.
Compassionate, empathetic, and patient-centered approach.
Ability to work effectively both independently and as part of an interdisciplinary team.
Strong documentation and follow-up skills.
