Resident Assessment & Care Plans: Conduct intake interviews, evaluate psychosocial needs, and develop comprehensive, individualized care plans in collaboration with the interdisciplinary team.
Discharge Planning: Coordinate the safe transition of residents back to the community or to other care levels, arranging home health services, durable medical equipment (DME), and follow-ups.
Grievance Management & Rights: Uphold residents' rights, investigate complaints, and monitor grievance logs to ensure timely and documented resolution.
Compliance & Auditing: Maintain survey readiness by ensuring accurate documentation (including MDS entries) and adherence to federal and state regulations.
