Schedule: Day Shift | On-site
We are seeking a licensed Social Worker to serve as Director of Social Services. This role is responsible for ensuring residents receive medically-related social services in compliance with Maryland COMAR regulations and federal CMS requirements, while supporting residents’ psychosocial well-being, discharge planning, and quality of life.
Regulatory & Licensing Requirements (Required)To meet Maryland SNF regulations, candidates must meet the following:

Active Maryland Social Work License, one of the following:
LBSW
LMSW
LCSW
LCSW-C
License must be in good standing with the Maryland Board of Social Work Examiners
Experience or demonstrated competency in long-term care / skilled nursing
Ability to comply with COMAR 10.07.02.30 and 42 CFR §483.15
Applicants without a Maryland social work license will not be considered.
Essential Duties & ResponsibilitiesProvide and coordinate medically-related social services to meet residents’ psychosocial, emotional, and behavioral health needs
Complete and maintain initial social history assessments within required regulatory timeframes (including within 7 days of admission)
Participate in interdisciplinary care planning, MDS coordination, and care conferences
Lead and support discharge planning, including coordination with families, hospitals, and community resources

Provide counseling and support to residents and families regarding:
Adjustment to illness or placement
Advance directives
Resident rights and grievance processes
Ensure accurate, timely, and survey-ready documentation
Maintain confidentiality and provide services in a private and appropriate setting

Ensure compliance with:
Maryland COMAR regulations
CMS Conditions of Participation
Resident Rights requirements
Serve as the facility lead for social services during state and federal surveys

Prior experience in:
Skilled Nursing Facilities (SNF)
Long-Term Care
Sub-acute or post-acute care

Familiarity with:
MDS process
Care planning systems
Maryland survey process
Strong communication, documentation, and organizational skills
Strong leadership and interdisciplinary support
Regulatory-focused environment with survey preparedness emphasis
