Director of Social Work (DSW)

A skilled nursing facility in Cooperstown, NY is seeking an experienced Director of Social Work (DSW) to lead our Social Services department and support resident well-being in a collaborative, supportive environment.

Responsibilities:

• Oversee all Social Services operations

• Lead discharge planning and care coordination

• Support residents, families, and interdisciplinary teams

• Ensure compliance with state and federal regulations

• Mentor and develop Social Services staff

Qualifications:

• Bachelor's Degree in Social Work or related field (MSW preferred)

• Leadership experience in a skilled nursing or healthcare setting (required)

• Knowledge of Medicare, Medicaid, and discharge planning

• Strong communication and organizational skills
