Social Services Designee

Garden Park Care Center is looking for a compassionate, organized, and resident-focused Social Services Designee to join our team! If you're passionate about advocating for residents, supporting families, and helping create a positive long-term care experience, we'd love to hear from you.

• Support residents and families throughout their stay

• Assist with admissions, discharges, and care planning

• Coordinate care conferences and communicate with interdisciplinary teams

• Connect residents and families with community resources

• Advocate for residents' rights and emotional well-being

• Maintain accurate documentation and ensure regulatory compliance

• Long-term care or skilled nursing experience preferred

• Previous social services or healthcare experience preferred

• Strong communication and organizational skills

• Compassionate, professional, and resident-centered approach

• LSW a plus, but not required
