Nursing Concierge

Job Summary:

Dry Harbor Nursing & Rehabilitation Center, a 360-bed skilled nursing facility dedicated to delivering exceptional care to our patients and residents, is seeking a highly skilled and compassionate Nursing Concierge to join our team. As a vital member of our front-line staff, you will play a critical role in ensuring a seamless transition for our residents and families, while providing exceptional customer service and clinical support. Responsibilities:

• Greet and register patients, families, and visitors, while maintaining a welcoming and professional atmosphere
• Process and coordinate admissions, discharges, and transfers, ensuring accurate and timely documentation
• Provide emotional support and guidance to residents and families, addressing their concerns and needs
• Collaborate with the healthcare team to ensure a comprehensive care plan is in place, addressing the unique needs of each resident
• Conduct initial assessments and identify potential issues, escalating concerns to the nurse manager or other appropriate personnel as needed
• Maintain accurate and detailed records, adhering to industry standards and regulations
• Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures

Requirements:

• Minimum of 3 years of experience in a healthcare setting

• Strong clinical knowledge and nursing skills

• Excellent communication and interpersonal skills

• Ability to work in a fast-paced environment with multiple priorities
