MDS Coordinator (RN/LPN)

Silvercrest of Urbana is seeking a dedicated and detail-oriented MDS Coordinator (RN or LPN) to join our clinical leadership team. If you have a passion for accuracy, regulatory compliance, and ensuring residents receive the highest quality of care, we'd love to hear from you!

As our MDS Coordinator, you'll play a vital role in coordinating resident assessments, maximizing reimbursement, supporting interdisciplinary care planning, and ensuring compliance with state and federal regulations.

What You'll Do
  • Coordinate and complete MDS assessments in accordance with CMS and Ohio regulations.

  • Ensure timely and accurate completion of all required assessments and submissions.

  • Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.

  • Participate in resident care plan meetings and help develop individualized plans of care.

  • Monitor clinical documentation to support quality outcomes and reimbursement.

  • Review Medicare and insurance documentation for accuracy and compliance.

  • Assist with survey readiness and regulatory compliance initiatives.

  • Analyze quality measures and work with leadership to improve resident outcomes.

  • Educate and support staff regarding documentation requirements and MDS processes.

Qualifications
  • Current Ohio RN or LPN license in good standing.

  • Previous MDS experience in a skilled nursing facility preferred.

  • Working knowledge of RAI process, PDPM, Medicare, Medicaid, and CMS regulations.

  • Strong assessment, organizational, and critical thinking skills.

  • Excellent communication and collaboration abilities.

  • Experience with electronic medical records preferred.
