Complete Care is seeking an experienced and relationship-driven External Case Manager to support our Madison Region. This position serves as a vital link between hospitals, healthcare providers, and our skilled nursing facilities by developing referral relationships, coordinating admissions, and ensuring seamless transitions of care.
This is an excellent opportunity for a healthcare professional with strong hospital liaison, admissions, or healthcare business development experience who enjoys building partnerships and making a meaningful impact on patient outcomes.
ResponsibilitiesDevelop and maintain strong relationships with hospital case managers, discharge planners, physicians, and healthcare partners throughout the Madison region.
Coordinate referrals and facilitate efficient admissions into Complete Care skilled nursing facilities.
Partner with facility Admissions Directors and leadership teams to ensure timely communication and exceptional customer service.
Conduct regular hospital and community visits to strengthen referral partnerships and identify new opportunities.
Serve as the primary external liaison throughout the referral and admission process.
Collaborate with clinical and operational teams to ensure smooth patient transitions.
Track referral activity, maintain accurate documentation, and monitor admission trends.
Represent Complete Care professionally at networking events, healthcare meetings, and community outreach initiatives.
Bachelor's degree preferred or equivalent healthcare experience.
Previous experience as a Hospital Liaison, Admissions Director, Admissions Coordinator, External Case Manager, or in healthcare business development strongly preferred.
Skilled nursing, rehabilitation, hospital, or long-term care experience highly desirable.
Proven ability to build and maintain professional relationships with referral sources.
Excellent communication, organizational, and time management skills.
Valid driver's license and willingness to travel throughout the Madison region.
