Northbrook Behavioral Health Hospital located in Blackwood, NJ is seeking a full-time Behavioral Health Discharge Planner as part of the Social Work Dept.
Monday-Friday (1st Shift) No Weekends
The ideal candidate will have a bachelor’s degree in social work and the MSW OR CSW certification is required.
This position involves meeting with patients, speaking to families and outside agencies, documenting in the electronic medical record, and assisting LSWs and LCSWs in making referrals. This is a fast-paced environment and requires incredible attention to detail. The Discharge Planner will work independently, when necessary, as well as part of the Social Work team under the direction of the unit’s Social Workers.
