A behavioral health provider in Phoenix is seeking a dependable Behavioral Health Case Manager to support adults receiving behavioral health services.

The Case Manager helps members stay connected to treatment, appointments, community resources, benefits, housing, and other services needed to support stability and progress toward treatment goals.

Responsibilities

  • Coordinate care and services for assigned members
  • Facilitate Groups
  • Assist members with scheduling and attending medical, behavioral health, and community appointments
  • Connect members with housing, benefits, transportation, employment, and other community resources
  • Complete progress notes, assessments, service plans, and other required documentation
  • Monitor member progress and follow up on identified needs
  • Participate in treatment planning and care coordination with the clinical team
  • Communicate with providers, families, health plans, and community agencies as appropriate
  • Assist with discharge planning and transitions between levels of care
  • Identify changes in member needs, behaviors, or safety concerns and report them appropriately
  • Maintain accurate records and meet required documentation timeframes

Qualifications

  • Behavioral health, case management, social services, or related experience preferred
  • Experience working with adults with mental health, substance use, or SMI needs preferred
  • Strong organization, communication, and documentation skills
  • Comfortable using electronic health record systems and computers
  • Valid driver's license and ability to transport members as needed
  • CPR and First Aid certification
