Nurse Practitioner – Family Medicine
Location: San Jose, CA
Salary: $158,000–$185,000/year | Full-Time
About the Role:
Facility seeking a compassionate Family Nurse Practitioner to provide primary care services to underserved communities in a mission-driven FQHC setting.
Key Responsibilities:
- Provide primary care, diagnosis, treatment, and preventive services.
- Conduct exams, develop treatment plans, and manage medications.
- Educate patients and monitor outcomes.
- Collaborate with physicians and multidisciplinary teams.
- Maintain accurate EHR documentation and coordinate referrals.
Requirements:
- Master’s or Doctorate from an accredited NP program.
- Active, unrestricted California NP license.
- Board Certified.
- Active, unrestricted DEA license.
- Current BLS certification.
- 2+ years of primary care experience required.
- FQHC/community health experience preferred.
