Social Service Director

A respected post-acute care provider in the southwest suburbs is looking for a Social Services Director to take ownership of resident well-being. This facility places high value on teamwork, integrity, and innovation in resident care.

What You'll Do:

• Manage the psychosocial care process for residents from admission to discharge

• Facilitate care conferences and individualized service plans

• Guide residents and families through transitions, concerns, and referrals

• Supervise social services staff and collaborate with interdisciplinary teams

• Uphold regulatory standards and quality benchmarks

The Ideal Candidate Has:

• A Bachelor’s degree in Social Work or Human Services field (LSW or LMSW a plus)

• At least 2 years of experience in long-term care or healthcare social work

• Deep understanding of discharge planning and case management

• Empathy, professionalism, and a solutions-focused mindset

• Prior leadership experience strongly preferred
