Now Hiring: Admissions Coordinator

Responsibilities:

  • Serve as the primary point of contact for individuals seeking admission to our recuperative care facility.

  • Conduct initial screenings and assessments to determine eligibility for admission.

  • Coordinate with healthcare providers, social service agencies, and other stakeholders to facilitate referrals and admissions.

  • Collect and review necessary documentation, including medical records and insurance information, to complete the admission process.

  • Communicate effectively with residents and their support networks to address questions and concerns and provide assistance as needed.

  • Collaborate with the interdisciplinary team to develop individualized care plans and ensure continuity of care.

  • Maintain accurate and up-to-date admission records and documentation in accordance with regulatory requirements.

Qualifications:

  • High school diploma or equivalent required; associate or bachelor's degree preferred.

  • Compassionate and empathetic attitude towards individuals from diverse backgrounds, including the homeless population.

  • Previous experience in a healthcare or customer service role preferred.

  • Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations.

  • Excellent organizational skills and attention to detail.

  • Ability to work independently and prioritize tasks in a fast-paced environment.

  • Proficiency in computer skills, including Microsoft Office Suite and electronic medical records (EMR) systems.

  • Compassionate and empathetic demeanor, with a commitment to providing patient-centered care.
