Responsibilities

The RN Minimum Data Set (MDS) Coordinator is responsible for accurate and timely completion and transmission of MDS assessment of all residents in facility. Participates as an active member of the interdisciplinary team. Collaborates with the DON to provide accurate information for quality care of the resident, including audits and care conferences.

Qualifications

Education
• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
• Completion of MDS Assessment training class within 6 months of entry into job required
• Bachelor's Degree in Nursing preferred
Experience
• 2 years of long term care experience required
• Experience in completion of MDS assessments preferred
Licenses and Certifications
• RN - Current and valid Registered Nurse license to work within the state of California required
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
