Job Summary

We are seeking a dedicated and compassionate Nurse to join our team at Meadow Falls of Finneytown in Cincinnati, OH. As a Nurse at our skilled nursing facility, you will provide high-quality patient care and support to our residents, promoting independence, dignity, and well-being.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and medical treatments as prescribed by physicians
• Monitor and report patient conditions, vital signs, and any changes to healthcare providers
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with interdisciplinary teams to ensure comprehensive care and achieve optimal outcomes
• Maintain accurate and detailed records of patient care, including progress notes and medication administration
• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes

Requirements:

• High school diploma or equivalent required, with a degree in Nursing (or related field) preferred
• Current LPN/LVN license in the state of Ohio
• Ability to lift, move, and transfer patients weighing up to 250 pounds
