Responsibilities
• Maintain and organize resident medical records in accordance with facility policy and regulatory requirements
• Ensure resident charts, forms, and documentation are complete, accurate, and properly filed
• Assist with record requests, audits, and compliance-related documentation
• Support admissions, discharges, and transfers with appropriate paperwork and record management
• Schedule and coordinate resident medical appointments with physicians, specialists, and outside providers
• Coordinate and assist with transportation scheduling to ensure residents arrive safely and on time for appointments
• Safely transport residents to and from scheduled medical appointments using the facility vehicle as needed
• Assist residents during transportation while promoting safety, dignity, comfort, and excellent customer service
• Communicate appointment schedules and transportation needs with nursing staff, residents, families, providers, and transportation vendors
• Track appointment calendars, transportation schedules, and follow-up appointments to support efficient daily operations
• Serve as the facility receptionist by greeting visitors, answering and directing incoming phone calls, and assisting residents, families, vendors, and guests in a professional and welcoming manner
• Provide administrative and clerical support to the leadership team and other departments as needed
• Maintain the cleanliness and readiness of the facility transportation vehicle and promptly report maintenance or safety concerns
• Maintain confidentiality and protect resident information in accordance with HIPAA regulations
• Work collaboratively with nursing, therapy, social services, admissions, and leadership teams to ensure excellent communication and continuity of care
Qualifications
• Prior healthcare, medical records, scheduling, receptionist, or administrative experience preferred
• Long-term care or skilled nursing experience preferred
• Strong organizational and multitasking skills with excellent attention to detail
• Excellent communication and customer service skills
• Ability to maintain confidentiality and handle sensitive information professionally
• Experience with electronic medical records and Microsoft Office preferred
• Valid driver's license with an acceptable driving record required
• Ability to safely transport residents and assist them during appointments and community outings as needed
• Reliable, dependable, and able to work independently while supporting a team environment
