Job Description
Avir Trilogy is seeking a Social Services Director to join our team!
We're looking for a Social Services Director who can sit with a resident, understand what's actually going on, and then do something about it. You'll identify social and emotional needs across the building and provide medically related social services that support the highest practical physical, mental, and psychosocial well-being of every resident we serve.
The work runs from the first conversation to the last. You'll handle admissions inquiries and tours, complete social service assessments that drive each individualized care plan, coordinate outpatient appointments and transportation, and support residents through transitions home or to another facility.

What You'll Do:
Complete social service assessments on each resident and translate them into individualized care plans.
Prepare quarterly social service progress notes for every resident.
Conduct admission tasks including phone and in-person inquiries, facility tours, and completion of the admission contract.
Support residents transitioning home or to another facility.
Coordinate outpatient appointments, including transportation.
Help residents meet personal needs by connecting with families or arranging the weekly shopping trip.
Attend resident council meetings, care plan meetings, and other facility committees.
Prepare the bi-monthly resident newsletter.
Build and manage the annual social services department budget.

Qualifications for Social Services Director:
Bachelor's or Master's degree in Social Work. Texas social work licensure required.
Two years of experience working with geriatric populations.
Valid driver's license.
Ability to work independently, exercise sound judgment, and remain calm under pressure.
Patience, tact, and a positive attitude with residents, families, and staff.
Comfort working with residents who are ill, disabled, elderly, or at times upset.
Ability to move intermittently throughout the workday.
